The outpatient therapy program at Centre Care is designed to meet each individual's goals as part of their rehabilitation plan. In coordination with the nursing team and the patient's physician, our team will assist residents to meet their therapy goals. Patients can continue receiving therapy services following discharge, or, outpatient therapy can be provided without having been a resident or patient at Centre Care.
