Centre Care, a brand-new 240-bed skilled nursing facility will offer traditional long-term care, as well as three specialty neighborhoods that are in high demand for Centre County residents.

High Acuity Care

For patients with complex medical conditions

High Acuity Care

The high-acuity floor of 14 semi-private rooms and 2 private rooms will take care of 30 residents with complex medical conditions who have been or need discharged from the hospital but require time to adjust to new care demands and routines before returning home.

Memory Care

For patients with Alzheimer’s, dementia and other types of memory loss

Memory Care

The memory care neighborhood will serve those who will develop Alzheimer’s disease or another form of dementia and require more care than a family can provide. Patients will live in a more home-like environment, experience less anxiety and live happier lives.

Short-term Rehab

For patients in need of care for only a little while

Short-term Rehab

The short-term rehabilitation floor will serve the patients benefiting from advances in joint replacement and other orthopedic treatments, some of whom will require intensive physical therapy and time to adjust to new care routines before returning home.
